A person who forgets entirely, prays without wudu and remembers afterwards commits no sin, but the prayer is treated as void and must be offered again after performing wudu. The Prophet (peace be upon him) said that no prayer is accepted without purification (Sahih Muslim, hadith 224). Al-Fatawa al-Hindiyya gives the same ruling.
Praying without wudu deliberately is a grave sin, and the difference between forgetting and intention matters.
If someone suspects during prayer that their wudu has broken, they should not break off. In Sahih al-Bukhari (hadith 137), the Prophet told a man not to leave his prayer until he heard a sound or noticed a smell. Certainty is needed; doubt alone is not enough.
If there is doubt about whether one has wudu at all, the principle is that certainty is not removed by doubt. Someone sure of having performed wudu and only doubtful of having broken it is still in wudu (Ibn Abidin, Radd al-Muhtar). In the opposite case, remembering that it broke and doubting whether it was renewed, wudu must be performed again.
If doubt arises during wudu about whether a limb was washed, it is better to wash it when this happens rarely. Someone troubled by such doubts often should ignore them and carry on. For a specific question, it is best to ask a reliable scholar.
